Physical
This must be completed and signed by a licensed medical provider. You can upload a physical used for school, or we’ve provided a template that can be filled out.
Your child must have a current physical from within the past year to attend camp.
Immunization Record
Required immunizations:
Diphtheria, Tetanus and Pertussis
Polio
MMR
Varicella
Hepatitis B
*If you enroll after this date, we will reach out directly to discuss your personal camp preparation timeline.
Prep Checklist
Complete all required forms by May 1
Set aside time to review camp policies and the code of conduct together with your child
Write a letter before camp begins so your camper has mail waiting during the first few days
Prepare any medications and send them to camp one week before your session begins
Order any camp clothing (type BLAC on the homepage)
Clearly label all luggage with your camper’s first and last name and sign up for bag shipping if needed. Label all clothing and equipment with your camper’s first and last name.
Health Care
Your child’s health and safety are incredibly important to us. Berkshire Lake Arts Camp is licensed by our Local Board of Health and follows all Massachusetts Department of Public Health regulations. Many of the requirements below are state-mandated, and we appreciate your help in keeping our community healthy.

We have a registered nurse on-site 24/7 to care for campers throughout their stay. Campers are encouraged to tell their counselors if they don’t feel well, and our counselors are trained to notice when something seems off.
Our nurse works under standing orders that are reviewed annually by a physician.
If a camper shows symptoms of illness, she will be brought to the Health Center to be evaluated. Families are kept informed about any important health concerns, including:
Fevers or vomiting
Strains or sprains
Ongoing issues like headaches or stomachaches
Overnight stays in the Health Center
Any need for off-site medical care
We are about 15–20 minutes from urgent care and a hospital if needed. If your child ever needs off-site care, you will be notified before she leaves camp. In an emergency, families are contacted immediately.

All campers receive a health check on the first day of camp, including a head lice inspection.
To help prevent the spread of illness, please:
Check your child for head lice three weeks before camp and again right before departure
Notify us if anyone in your household has been treated for lice within the month before camp
If a camper shows signs of any communicable illness while at camp, she will be evaluated by our staff and cared for. This may include resting, temporary separation from group activities, increased monitoring, or off-site medical care. Our goal is always to care for the individual camper while protecting the health of our entire camp community.

All medications must be stored in the Health Lodge and administered by our nurse. This includes prescription medications, vitamins, melatonin, and gummies.
If your camper will take prescription medication at camp:
Complete the Camper Medication Form
Send medications in their original, labeled containers
Doses and timing must match the pharmacy label exactly
Important Notes:
We are unable to administer medications that differ from the label instructions or are not in original packaging.
Please mail all medications to camp one week before your session begins so our nurse can review them in advance.
If your child uses an inhaler or EpiPen, we recommend sending two if possible.

Our Health Lodge is stocked with an array of basic over-the-counter medications. You do not need to send OTC medications to camp unless your child takes them daily.
In the Health History form, you will indicate which OTC medications you approve for your camper to receive while at camp.
